Appointments are conducted virtually through our secure patient portal.
All new patients begin with a comprehensive initial consultation. During this visit, we will review your symptoms, medical history, health goals, and prior testing (if available). Together, we will determine whether Direct Primary Care, Metabolic & Hormone Health, a combination of both, or another service is the best fit for your needs.

Metabolic & Hormone Health
Ideal for patients experiencing fatigue, weight gain, brain fog, poor sleep, low libido, hormonal symptoms, thyroid concerns, or difficulty losing weight.
Initial Consultation: $50
Following your initial consultation, patients who need ongoing specialized care may enroll in the Metabolic & Hormone Health Membership. Patients who also want Feeling Alive Health to serve as their primary care provider may combine this with the Direct Primary Care Membership.
$100/month
Includes:

Direct Primary Care
Ideal for patients seeking ongoing virtual primary care, preventive care, chronic disease management, medication management, and acute care when appropriate.
Initial Consultation: $50
If ongoing care is recommended and you choose to enroll, your membership begins after your initial consultation. Membership fees are charged at the time of enrollment and renew monthly thereafter.

Includes:

Specialized hormone health, thyroid support, metabolic care, and medical weight management are available through the Metabolic & Hormone Health Membership. Patients may combine memberships if they would like comprehensive primary care and specialized care under one provider.

One-Time Lab Review Consultation
$60
For patients seeking a focused laboratory review without ongoing membership enrollment.
Includes:
Review of completed laboratory testing
Personalized recommendations
Discussion of potential next steps
This consultation does not include ongoing treatment, prescription management, or between-visit messaging.
Membership credit option: If you choose to join a monthly membership within 7 days of your lab review, your $75 fee will be credited toward your first month.

Acute Care Telehealth Visit
$60
Evaluation and treatment of select non-emergency conditions appropriate for telehealth.
Common conditions treated include: urinary tract infections, sinus symptoms, pink eye, rashes, minor skin infections, medication concerns, and other conditions appropriate for virtual evaluation.
Not intended for medical emergencies.
Comprehensive Care Under One Provider
Need both ongoing primary care and specialized metabolic or hormone care?
Many patients choose to combine the Direct Primary Care Membership with the Metabolic & Hormone Health Membership for integrated care under one provider.

Complete Intake Forms
Your confirmation email will include access to your secure patient portal and required intake paperwork. Appointments are confirmed once required forms and consents are completed.

Telehealth Requirement
You must be physically located in a state where Feeling Alive Health is authorized to provide telehealth services at the time of your appointment.
I currently provide telehealth services to patients located in Florida and Oregon.
